When doing the form . . . make sure and DO NOT DATE it . . . you only want t o date it when and if the client ftas.
I used the form with SS in 2003 and was told that they would honor it this time (charge was a felony) but for future reference SS does not accept anything dated more than 90 days. As the bonds person you can notarize the SIQ signature as you are not notarizing your own sig.
Shrek am still digging for the one for the UFAP. I have found the actual warrant stating that is was issued by complaint, but there is an actual compaiant form and I am trying to find the copy I have if not I will get the form #

If you notice, I have two places for signatures and information. That is because both the defendant and the cosigner have to sign.

I think this is one of the forms that I got from someone else a few years ago on this forum. I also have the defendant sign a bond extradition waiver even on misdemeanors. It doesn't say anything about LE or the courts, but that I or my agents can pick the person up and bring them back.

These two forms are something that many bondsmen do not use because they don't realize their importance until they need them. Those of us who have worked the streets know what we need. If we could only get the bondsmen trained, our jobs would be much easier :wink: .

I was correct. The authorization to release information form came from L. Scott Harrell. He shared it with those of us who asked for it during a similar discussion. I didn't find the topic, but I found the original form on my computer. So, make sure you credit him with authoring it.

I was using a shorter version when I was writing under a previous bondsman, because he wanted to have everything, applications, agreement, waivers, all fit on the front and back of one legal size page. I switched to this one when I changed agents. The only thing I am considering adding to it is a specific reference to HIPAA.

In the past I tried to get cooperation from Drug Rehabs and Mental Health treatment centers and got no cooperation even with a signed release of information, so I gave up on the "direct" approach.

The sneaky approach worked much better, I would contact the facility and ask to speak to the skip, I never asked if they were there but approached it as though I knew they were there and expecting to hear from me, I would explain that the warrant had been cancelled because the court was advised that the person was in treatment but they still needed to appear in court today to get a continuance so they could continue to get treatment, if they failed to appear a warrant would be issued which would certainly not be in the persons best interest, the Surety has arranged transportation via a Taxi company to get the defendant to court and back.

When the Taxi arrived at the courthouse I promptly deposited the skip into the county jail, end of story, no more worries about skip absconding from treatment program and continuing to run.

I got a nasty call from a Social Worker but that was the extent of the fall out.

LOL, it's all in the paperwork and the attitude, Chuck. Getting the defendant to cooperate is the best option.

BTW, I read your post on how the Taser was working out and forgot to respond. It seems to work great, but I didn't have it on me the first real chance I had to use it. That won't happen again!! I'm looking forward to lighting someone up when they try to run again :).

Kathy,

Please make sure you have some sort of plastic large garbage bags or cheap poncho and gloves in your car when carrying the taser. Some air freshener will be a good idea too.... :wink:

Remember when you hit someone with a taser, they lose all control of thier bodily functions...and you still have to transport them to the jail.
