Well other than the federal offices such as FBI, US Marshall and a few post offices, I haven't seem many lately.

Some local LE agencies have them on their websites. Those you can download and print out.

Also some bail recovery websites link to local wanteds.

Hey Bail Mo, I only used one years ago and I didnt keep the template. I think I made it up using MS Publisher. It wasn't anything special, just a photo, reward info and phone number.

Before I even handed them out around town, the skip found his way back into custody all by himself!
